IMPORTANT ANNOUNCEMENT

Due to summer vacation and the preparation for the Feast of Our Lady of Charity, the Virtual Program for Evangelization and Sacramental Preparation for Adults will be CLOSED from Monday, June 6th, 2022, through Sunday, September 18th, 2022. We’ll be accepting new applications starting Monday, September 19th, 2022.

Debido a las vacaciones de verano y los preparativos para la Fiesta de la Virgen de la Caridad, el Programa Virtual de Evangelización y Preparación Sacramental para Adultos estará CERRADO del lunes 6 de junio del 2022 hasta el domingo 18 de septiembre de 2022. Aceptaremos nuevas solicitudes a partir del lunes 19 de septiembre de 2022.

 

REGISTRATION PROCESS AND REQUIERED DOCUMENTATION

Those receiving the Sacraments of Baptism, Holy Eucharist, and Confirmation:

  • Completed and Electronically Signed Online Application.
  • Copy of Driver License or Photo ID.
  • Copy of Birth Certificate.

Those receiving the Sacraments of Holy Eucharist and Confirmation (already baptized roman catholic):

  • Completed and Electronically Signed Online Application.
  • Copy of Driver License or Photo ID.
  • Copy of Baptism Certificate.

Those receiving the Sacrament of Confirmation (already baptized roman catholic and have received the Holy Communion):

  • Completed and Electronically Signed Online Application.
  • Copy of Driver License or Photo ID.
  • Copy of Baptism Certificate.
  • Copy of First Communion Certificate.

*Those baptized in a non-Catholic Christian denomination, which baptism is accepted by the Catholic Church (baptism performed using trinitarian formula and water), must profess the Roman Catholic Faith within a Ceremony of Profession of Faith before receive the other sacraments of initiation.

*Other documents will be required before the ceremony of sacraments such as the Sponsor (Godparent) Eligibility Form and the Confirmation Name. The confirmation name must be the name of some saint according to our Catholic faith. For example: Usnavy is not yet a saint recognized in our faith, while Joseph is. The same is with names like Asia — America — Moon — Sun while Mary is.
